Prayer Blanket Request Form

Prayer blankets are typically made for those suffering from a serious illness, having surgery, grieving or depression.  Each knot on the blanket is tied while saying a prayer for the person.  If you know someone who needs a prayer blanket, please complete the information on this form and hit submit for it to be sent to the church office.

You will be responsible for delivering the blanket to the recipient.  Blankets will be ready after the last service on the Sunday following the request.  If you can't pick up the blanket at that time, it will be left in the usher's closet inside the wooden chest at the Shawver Welcome Center.  Requests must be received in the church office prior to close of business on Thursday of each week.
